This .pdf map contains the locations of regulated health care facilities in Colorado, including hospitals, outpatient care clinics, mental health clinics, community clinics, and rural health clinics.  The data used to create the map was collected from the Health Facilities and Emergency Medical Services Division of the Colorado Department of Public Health and Environment and was updated in December of 2012.  The size of this printed map document is 34 inches wide by 30 inches long.  The map also contains major roads and highways, as well as names of populated cities and towns.
